The Adopter(s) guarantees
that all members of the household are free of any past
and present charges of cruelty or abuse involving
animals and agree(s) to accept the following conditions
in order to adopt the dog described herein. The
adopter(s) agree(s) that the dog will receive all care
and attention necessary to ensure the health, well
being, and happiness of this dog.
Unless otherwise noted in this agreement, PPP-SPCA
believes this dog to be healthy and suitable as a pet.
However, since these are usually rescue dogs, PPP-SPCA
cannot guarantee the health and/or temperament of any
dog.
In order to provide an emotionally and physically
healthy environment for this dog, I
(we) agree to all of the following:
- This dog shall have access to the
adopter(s) on a regular basis to ensure socialization.
- Proper housing shall be provided at
all times to protect the dog from inclement or extreme
weather. This shall include providing climate control
of heating or cooling. If evacuation is called for,
the dog shall not be left in harm's way.
- No dog shall be left outdoors
without supervision except in a properly fenced yard.
Shade, water and shelter shall be provided at all
times. No dog shall be tethered for more than (1)
hour at a time and no more than three (3) hours in any
twenty-four (24) hour period. NO DOG SHALL BE ALLOWED
TO ROAM FREELY AT ANY TIME.
- This shall include notifying all
appropriate animal control/rescue agencies and
checking pounds and shelters in order to reclaim the
dog. The adopter agrees to pay any expense or fine
incurred due to such event.
- The dog shall have clean, fresh
water easily accessible at ALL times. Nutritional
food shall be provided sufficiently to maintain the
health of the animal.
- The dog shall be provided with
regular veterinary care and grooming to ensure its
health and comfort. This includes, but is not limited
to: keeping the dog free of fleas, ticks, matted fur,
eat mites, and internal parasites, including
heartworms. He or she shall be provided with
immediate veterinary care if injured or ill. All
vaccinations shall be kept current including rabies.
A rabies tag and required community licensing shall be
worn by this dog at all times including when the dog
in inside.
- The adopter(s) agrees this dog will
not be abused, used for attack, or any aspect of
animal fighting, will not be physically harmed,
treated cruelly, or subjected to any biological,
chemical, psychological, or other discomfort.
- The adopter(s) agrees that if the
dog is not sterilized prior to this adoption, the
adopter shall make arrangements to have the dog spayed
or neutered as soon as the dog's health or age
allows. If this dog is not currently sterilized, this
document will have attachments which the adopter must
also agree to.
- The adopter(s) agrees to keep this
dog for as long as it shall live. This dog shall not
be euthanized for any reason other than a veterinary
diagnosis of an incurable and painful illness or
injury. The ownership or custody of this dog may not
be transferred to any other person, business, or
organization without the express written
permission of PPP-SPCA. Otherwise, if the adopter(s)
cannot keep the dog for ANY reason, it must be
returned to PPP-SPCA.
- This adoption shall have a trial
period of ten (10) days After that the adoption
shall be considered permanent. If for any reason
(other than a violation of any terms of this
agreement) the adoption is cancelled, the adopter
shall notify PPP-SPCA and return the dog. Upon
verifying that the dog is in the same condition as at
the time of adoption, PPP-SPCA will refund the
adoption fee minus ten dollars ($10.00) transportation
fee.
- If at any time this dog is treated
in a manner not consistent with this agreement or if
any part of this agreement is violated, PPP-SPCA may,
at its discretion, reclaim this dog and the adopter
will be required to pay all costs associated with that
event.
By signing below, the adopter(s)
acknowledge(s) having read and understood all provisions
of this agreement.
